Evaluation Of Interleukin 6 Level Before Chemotherapy On Acute
Lymphoblastic Leukemia L1 Standard Risk And High Risk Patients

Abstract
Background Acute lymphoblastic leukemia (ALL) is a malignancy originating from clonal expansion of
lymphoid progenitors that have undergone neoplastic transformation at various stages of differentiation.
Interleukin-6 (IL-6) is a pleiotropic inflammatory cytokine produced by various cell types including T
cells, macrophages and stromal cells, in response to tumor necrosis factor-alpha (TNF-α) and interleukin-1
(IL-1). Objective To evaluate serum levels of IL-6 before chemotherapy in standard risk and high risk ALL
patients.Methods The research method was cross sectional study conducted in ALL patients who were
treated at RSUP Dr. Wahidin Sudirohusodo during April 2021 – June 2021. A total of 60 ALL patients were
included in the study. Serum IL-6 specimens were examined before the patient received chemotherapy.
Results The results showed that from 60 samples of ALL patients, 30 patients were standard risk and 30
people were high risk. Elevated levels of IL-6 before chemotherapy were found in both standard risk and high
risk ALL and significantly higher in the high-risk group compared to the standard risk group with P=0.022.
The cut-off point 64.23 ng/mL for high-risk ALL patients was obtained through ROC with sensitivity 63.3%,
specificity 63.3%, positive predictive value 63.3% and negative predictive value 63.3%. Adjusted Odds Ratio
2.983 with 95%CI 1.044 to 8.527. Conclusion IL-6 levels among high risk acute lymphoblastic leukemia
were higher than standard risk. Elevated levels of IL-6 were found in both standard risk and high risk ALL.

Serum Transforming Growth Factor - β (TGF-β) Levels on The Severity of
Retinoblastoma In Children

Abstract
Background Increased expression of TGF-β is a factor that plays a role in the growth of Retinoblastoma, thus
TGF-β levels are a parameter of the severity of Retinoblastoma disease. Objective To evaluate serum levels
of Transforming Growth Factor-β (TGF-β) before chemotherapy in patients with Retinoblastoma Grade III
and IV. Methods This study was an observational study with a cross sectional approach, carried out at Dr. dr.
Wahidin Sudirohusodo Hospital from January to November 2019. The study sample was pediatric patients who
were diagnosed with Grade III and IV Retinoblastoma and had not undergone chemotherapy. Patients who
have fulfilled the inclusion criteria then have a blood sample taken to check TGF-β levels. TGF-β level data
obtained were then analyzed using SPSS. Results A total of 38 patients participated in this study, consisted
of 13 Grade III and 25 Grade IV Retinoblastoma patients. The results showed that the mean TGF-β level of
Retinoblastoma Grade III was 1061 ng/L and Retinoblastoma Grade IV was 988 ng/L. Mann-Whitney U test
results showed no significant difference between the levels of TGF-β Retinoblastoma Grade III and IV, with
p value = 0.655 (p> 0.05). But TGF-β levels in both groups increased significantly above normal values (>
100 ng/L). Conclusion There was no significant difference in TGF-β levels in patients with Retinoblastoma
Grade III and IV. But TGF-β levels in both groups increased significantly above normal values, proving that
TGF-β has an important role in tumor cell development and is a parameter of the severity of retinoblastoma.
