Urinary Tract Infection in Neonates with Congenital Anomalies of Kidney and Urinary Tract (CAKUT)

Abstract
Background Urinary tract infection (UTI) is one of the most frequent infections among infants. The general
prevalence is 0.1–10% of which 0.1–1% is in term infants and up to 10% in preterm infants. Little guidance is
offered regarding febrile UTIs in those younger than 8 weeks of age. This special population may have special
characteristics that should be identified for the establishment of guidelines in the future. Objective The purpose
of this study is to elaborate investigation and management of UTI in neonates with CAKUT. Methods This
review was searched all known literature in the references to relevant studies. Results The signs and symptoms
of UTI in infants are non-specific, yet prone to develop into serious bacterial infection. Prematurity, low
birth weight, and uncircumcised are important risk factors of urinary tract infection in infants. Furthermore,
there is a possibility of urinary tract abnormalities as a background of the disease. Diagnosis of urinary tract
infection is based on positive urine cultures obtained from suprapubic samples or bladder catheter, in which
suprapubic aspiration of bladder is the most reliable technique of diagnosis. If urinary tract abnormalities
and UTI were left untreated, they will lead to a long-term complication such as high blood pressure and
kidney failure. Conclusion Management consists of eradicating acute bacterial infection, evaluate any possible
underlying structural kidney abnormalities, and prevent further kidney complication.
