Diagnosis and Management of Acute Rheumatic Fever in Children: A Case Series

Abstract
Background Acute rheumatic fever (ARF) is inflammatory condition that affecting different organs and
organ systems. In most cases, an appropriate treatment with antibiotics will prevent acute rheumatic fever.
However, diagnosis of ARF in children is challenging due to lack of specific symptoms. Objectives To
report current various clinical features of ARF based on jones criteria in children and its management. Cases
First case: a 8 years old girl, presenting with polyarthritis, fever, elevated acute phase reactants and normal
echocardiography. Second, third and fourth cases: 6 years old boy, 14 years old boy and 16 years old boy,
presenting with fever, arthralgia and carditis. Fifth case: a 13 years old boy, presenting with fever, arthralgia,
carditis, elevated acute phase reactants and prolonged PR interval electrocardiogram. Sixth and seventh cases:
3 years old boy and 12 years old boy, presenting with fever, arthralgia and carditis. All cases were found
elevated titter of antistreptolysine O (ASO). All cases are given antibiotic therapy as eradication of group of
α-streptococcal infection. Anti-inflammatory treatment (corticosteroid) and salicylates are given for six cases.
Conclusion Most common found symptoms are based on Jones Criteria of carditis (mayor criteria) was
obtained in 6 of 7 cases and fever (minor criteria) in all cases. Management of ARF includes antibiotic as
eradication of the group α-streptococcal infection, corticosteroid, salicylates, symptomatic treatment and
nutritional support.
