Relationship Between Leukopenia and Shock in Pediatric Patients with Dengue

Abstract
Background Dengue fever is a disease caused by dengue virus infection that can infect all age groups including
children. Dengue fever can cause shock due to haemorrhage, plasma leakage, and organ impairment which
can be fatal. Several studies show that there is a relationship between leukocytes and clinical manifestations,
but there are varying results. Objective To determine the characteristics of patients along with the relationship
between leukopenia and shock in pediatric patients with dengue fever at Jailolo General Hospital. Methods
A cross-sectional study in which data were taken from the medical records of pediatric patients with dengue
fever from November 2019 to March 2020. Results We collected 26 samples consisting of 13 males and
13 females, with an age range of 2-16 years. Symptoms of fever were found with loss of appetite (85%),
fatigue (65%), joint pain (46%), vomiting (19%), heartburn (15%), spontaneous bleeding (15%), and altered

mental status (4%). The leukocytes range are 1.8 x 10 - 11 x 10 mg/dL with mean leukocytes are 6.1 x 10

mg/dL. There were 16 cases with leukopenia, and 3 cases with shock. There is no significant relationship
between leukopenia and the incidence of shock in pediatric patients with dengue fever (P value > 0.05).
Conclusion The most common symptoms were fever with loss of appetite and fatigue. The finding of
leukopenia on hospital admission is not a predictor of shock in patients with dengue fever.

Encephalitis in Pediatric Patients with Severe Acute Respiratory Syndrome
Coronavirus 2 (SARS-CoV-2) Infection: A Case Report

Abstract
Background Many studies have shown that the predominant clinical manifestation of SARS-CoV-2 is
related to the respiratory symptoms. However, as the case numbers steadfastly increased, other symptoms
have been identified. Neurological manifestation has been recognized as one of the clinical manifestations
of SARS-CoV-2. Objective To report a case of a child with encephalitis as the neurological manifestation of
SARS-CoV-2 infection. Case A-3-year-old boy was referred to the hospital with chief complaint of febrile
seizure. Seizures occur twice with a duration of more than fifteen minutes each and had a characteristic of
generalized seizure. After the seizure, he tended to fall asleep and was difficult to wake up. No previous
history of seizures. He had fever for two days before admission. He also complained of vomiting and acute
diarrhea. On the first evaluation, his consciousness was stupor and he had fever, tachycardia, and tachypnea.
No meningeal sign and paresis found. His PCR test confirmed the diagnosis of SARS-CoV-2 infection. His
brain CT-scan did not indicate any pathological findings. However, according to neurologic expert, the case
was caused by encephalitis associated with SARS-CoV-2. After rehydration and bicarbonate correction, he was
treated with methylprednisolone, phenobarbital, and other symptomatic therapy. On the 5th day of treatment,
the state of consciousness had improved. He was able to be discharged on the 8th day of hospitalization.
Conclusion The case highlights that SARS-CoV-2 infection can lead to neurological manifestation. Proper
diagnostic and treatment at the right time may provide a better outcome.
