Coronavirus 2 Infection on Newborn Baby in RSUP Persahabatan:
A Case Report

Abstract
Background SARS-CoV-2 attacks almost all ages with the most cases in adults, but some reports have been
found in cases of newborns. Vertical transmission is still being studied in many countries. Objective To report
a confirmed case of COVID-19 on newborn. Case A neonate was born by cesarean section to a mother with a
positive rapid test PCR for SARS-CoV-2. The neonate was born well, APGAR score 9/10, birth weight 3700
grams. On the 18 hours of age, the IgM SARS-CoV-2 was nonreactive. On the age of 19 hours, the neonate
had brown vomiting and shortness of breath. Laboratory results showed an increase in monocytes, APT test
presented the baby's blood, X-rays showed transient tachypnea of the newborn and intestinal thickening.
The neonate's rapid test PCR SARS-CoV-2 swab was carried out at 24 hours old and was repeated 24 hours
after, both showed positive result. The baby was treated with second line of antibiotics. On the third days
hospitalization, the symptoms disappeared. Laboratory evaluation showed an increase of CRP, 2 grade

toxic granulation, segmented, vacuolization, the bilirubin levels showed 12.6 mg/dL, so antibiotics was
replaced and light therapy was performed. Until the 5 day, laboratory results showed the percentage of
th
monocytes tends to increase, lymphocytosis was accompanied by atypical lymphocytes, thrombocytopenia,
and negative blood culture. The swab performed at the age of 12 days was still positive. On the 16 days of
age, the IgM SARS-CoV-2 showed reactive IgM and IgG SARS-CoV-2. The swab performed at the age of
28 days were negative and the neonate was in a good condition. Conclusions SARS-CoV-2 infection can
be found in neonate from mothers infected with COVID 19 with clinical features of digestive disorders,
thrombocytopenia with atypical lymphocytes, and monocytosis.
